* Act 34 (State Police) - Free
* Act 154 (Child Abuse) - Free
* Act 114 (FBI Fingerprint - PA resident less than 10 years) - $27.00
* Volunteer Affirmation Form (PA resident more than 10 years)
* Volunteer Application
* Volunteer Acknowledgement & State of Confidentiality Form
